Since it is taking over your life and you are asking for help, I also suggest you talk to a psychotherapist and psychiatrist about how bad this is and what your options are as far as treatment. Also it could be a symptom of something else.
If it really is a phobia that you want to get rid of, going through desensitization, medicine, and therapy may possibly alleviate matters for you. You can also try looking up desensitization therapy in a google search.

Edit: The inability to move...sounds like (night) hag syndrome, night terrors, night or dream paralysis....If you have a sleep disorder and not enough quality REM sleep, that could cause you to have problems in the day too.
I have an in-law who I was told has problems sleeping and she has awful things she sees in the day and she screams.
Please go to a psyshiatrist or diagnostic center to get all this checked out.

I don't believe that the night paralysis necessarily lasts a few minutes maximum. But if that is the case then it just seems to last longer for some reason. I know this is all frightening. I have heard and read stories about this stuff: one thought she was being held down by a big furry black or brown thing, another thought or felt the presence of a dark sinister man in the room, I have had fear so enormous and felt an evil presence whispering in my ear (I still to this day don't know if I was asleep or half awake) that I could not move the fear gripped me and I said prayers (still don't know if I said them in a dream or real life?). When I was a teen I had nightmares and night terrors, there's a difference, and I had them both. I was going through stress, depression, bullying, and watched horror movies and read horror books. Even after a lot of this stopped, I still have problems from time to time. Like when I lost my mom and baby, I had nightmares.
If this night paralysis and other stuff you mention is severely interferring with your life, you need to see a professional to help you, not people on yahoo answers. We can only do but so much to help you. I could give you a lot of suggestions, but the best thing to do is contact someone who can really see you person to person and make a correct diagnosis. It is very difficult to diagnose someone on a computer, because you may need testing done to see if the quality of your sleep is so diminished that you are having the other problems while you are awake.

I have a fear of germs and spiders and of getting a life-threatening infection from germs. I had the fear of germs before I ever was Dx with a life-threatening infection. The doctors said I was becoming septic, and once I did, there was nothing they could do for me. I do not have the infection (as far as I know) now, but still think it can happen. I think what I have is OCD. You might want to look that up in wikipedia if you believe your fear is irrational and you have repetitive patterns or thoughts where you go through rituals to make yourself feel better... Also you might want to look up anxiety or anxiety disorder, even if you may (not) have this.
